Nebraska Creative Districts – Public Art Entry Form

This form can be used by artists and organizations to submit artwork to the Public Art Archive as part of the Nebraska Creative Districts Project.

What is “Public Art” for the purpose of this project? Public art is artwork that is freely accessible to the public. In most cases, public art encompasses artwork found outside in public space or in public buildings where the doors are most often unlocked and open to the public (such as libraries, municipal buildings, post offices, etc.)

If you need to submit changes to previous submissions, please follow the process for editing existing artworks on the Public Art Archive on our Contact page .

  • Select the option that best fits your needs. Select Add a New Record if the artwork has not been submitted to the Public Art Archive or Edit an Existing Record if you need to make changes to a previous submission.
  • Public-Facing Data

    The data input into the following fields will be used to create artwork records on the Public Art Archive. These fields will be public-facing.
  • Please copy and paste the URL of the artwork record published on the Public Art Archive.
  • If you answered yes to the question above, please list the Creative District the artwork belongs to below
  • List the year the artwork was created.
  • If temporary, list the dates the artwork was/is on view to the public. *Note: If no longer on view to the public, the Public Art Archive will include an “Off View” artwork tag on the public portal.
  • The organization that commissioned, manages, and/or is responsible for the artwork. This may include multiple agencies.
  • The organization or person that owns the artwork. This may be the same entity (or multiple entities) as the Collection. In most cases, the artwork owner owns the building or manages the public space where the artwork is sited.
  • Select from options on dropdown list that best describe the artwork
  • List any additional artwork types that best describe the artwork
  • Detail the materials used in the artwork
  • Select from options that best describe the theme of this artwork. Examples include terms such as: animals, climate change, education, figures (representations), landscapes (representations), neighborhood projects, recycling, sports, etc.
  • List the complete street address of the artwork. Include latitude and longitude if possible. For details on how to find the latitude and longitude, visit this resource.
  • Select a placement type from the dropdown list to describe the site of the artwork
  • Paragraph format to describe the visual elements of the artwork, the subject matter, the story behind the artwork, the artwork site, and/or the artist biography.
  • Use this space to detail any changes to your existing record on the Public Art Archive. Please be as specific as possible.
  • If you’d like to add new images to your record, please upload here. Limit 5 MB per image. If you have additional materials such as PDFs, you may upload here as well. Please make sure to list the image credits in the Request Edits to Existing Records on the Public Art Archive field.

      • Any additional information you’d like to share about the project. This can include links to streaming video content, press releases, links to publications where the artwork/project is mentioned, etc.
      • Internal Fields

        The data collected in the following fields will be internal-facing only. No data in this section will be viewable by the public. This information will be available to Nebraska public art organizations, upon request to the Public Art Archive team as Creative District administrators, to aid with inventory reports on your collection and to track budgets, maintenance needs, condition reports, and other details about your collection.
      • What is the total budget associated with this project?
      • Upload budget forms and documents to share amongst Nebraska Creative Districts. If provided, this will be shared with other Creative Districts upon request to help improve understanding on how projects are funded.
